FIRST STEPS TO HOME HEALTH CARE

Step 1. Assessment

Your family member will receive a complimentary assessment from one of our registered nurses to identify their goals, specific needs, and the appropriate level of care required. To facilitate a comprehensive evaluation, our Director of Nursing, Director of Care, and a Company Representative will all visit together in a single appointment, saving you time and alleviating any potential stress.

Step 2. Planning

Once the evaluation is complete, our nurse will develop a personalized care plan tailored specifically to your loved one’s needs. We recognize the importance of healthy social interactions, so our care plan also prioritizes attending to the psychosocial well-being of your loved one.

Step 3. Match

Following this, we carefully select a caregiver who aligns with your loved one’s individual needs and personality. We understand that the relationship between the caregiver and your loved one is vital, and our program is designed to foster these meaningful connections. We ensure that the bond between the caregiver and your loved one creates a nurturing and supportive environment.
